- Provide and/or coordinate services for no more than 12 children.
- Must have a Bachelor's Degree in Behavioral, Social, Sciences, Social Worker, Psychology, or Childhood Education with 2 years of paid work experence in the field of Social Services or Human Services.
- Manage, maintain and upkeep of all resident records, all medical contacts, counseling contacts, and doctor’s order.
- All documentations are clear, accurate, and completed by all staff members.
- Develop, complete, review, process, file, and submit documents and reports.
- Writing assessments and Individualized Service Plans, Progress Reports, Intake, and Discharge Summaries for each child.
- Provide and coordinate ancillary and Social Services for the child.
- Knowledge of appropriate developmental treatment programs, evaluation, observation techniques.
- Ensure all information is updated, accurate, and recorded in GA+ SCORE/SHINE daily and DJJ.
- Complete behavioral management forms on each child (crisis intervention & safety plans).
- Attend meetings, IEPs, probation officers, and work with case workers, etc.
- Ensure that residents files are kept confidential and secure at all times.
- Ensure that the educational, medical, emotional, and social needs of the children are met.
- Schedule and transport residents to all scheduled appointments (medical, dental, eye glass, meetings, court etc.)
- Monitor contact between the childbirth family, DFCS, DJJ and all other pertinent providers.
- Provide oversight of services to children and families in a home-like setting.
- Make sound decisions in the Director's absence, not limited to assigning staff duties & providing day to day oversight for the residents.
