Overview
The Herefordshire Single Point of Access, known as the Community Referral Hub (CRH), connects patients and professionals to the right community support, every time, enabling both planned and unplanned needs to be met through effective navigation, promoting timely intervention and admission avoidance.
We are looking to enhance our offer of urgent care and admission avoidance support for our district nursing teams and are looking for enthusiastic and experienced nurses to join our team. We are looking for colleagues with a background in district/community nursing, who are suited to work in a fast‑paced environment, are able to make decisions and who are able to provide clinical nursing advice and support.
The role is mainly office based but depending on service needs you will be expected to work clinically at times.
You will be based in the Community Referral Hub which is a truly integrated and multi‑organisational service. The Hub currently manages referrals and patient queries for the District Nurses, Urgent Response Service, Virtual Wards, Virtual GP, IVOPAT, Therapies (OT/PT) and Falls Support. You will be offered comprehensive induction support and ongoing training and support to develop the role.
ResponsibilitiesIn this exciting new post you will support the clinical triage and streaming of referrals in our Community Referral Hub. Taking referrals directly from professionals where its more effective they speak directly with a clinician, undertaking remote assessments over the phone as required and determining, in collaboration with the patient/carer/professional, which service is best placed to meet their need to avoid a crisis and/or admission.
A robust understanding and knowledge of the 9 core clinical conditions is a must and being able to work collaborative across the various community teams is key to the success of this role.
