Home Care Assistant

Rainbow Care Solutions is a long-established, CQC-registered home care provider with over 20 years experience. Were recruiting Full-Time Home Care Assistants (37.5 hours/week) to support our Discharge to Assess (D2A) / Reablement service across Herefordshire. This role supports people returning home from hospital to recover safely and regain independence doing with, not doing for.
• Location Herefordshire (community-based) What youll be doing
• Supporting people at home after discharge with reablement-focused care Assisting with personal care where needed, while encouraging independence
• Supporting daily living skills (washing, dressing, meal prep, mobility, routines) Following care plans and recording notes accurately
• Reporting changes in condition, concerns, or risks promptly
• Working respectfully with families and professionals as part of a wider team
• What were looking for Caring, reliable and professional approach
