Wye Valley NHS Trust is seeking a Senior Occupational Therapist (Band
6) to join the Discharge to Assess (D2A) service in Hereford. You will assess patients in their own homes after hospital discharge and develop personalised rehabilitation plans to promote independence and safe discharge.
You will work within a MDT, coordinating with therapy teams, social care and primary care to optimise patient outcomes and reduce length of stay.
