Main duties of the job

Youll playa vital role in offering expert clinical care, managing both acute andchronic conditions. This is an ideal opportunity for someone passionate about leadership in primary care and keen to make a real difference in community health.

Hours ofwork Monday to Fridaytriaging and seeing patients,reviewing and actioning laboratory results,signing prescriptions (if you are an independent prescriber), and supporting urgent clinical queries.

You'll havea mix of face-to-face and telephone consultations, and autonomy to manageminor illnesses, acute presentations, and long-term conditions within yourscope of practice. The role will be supported by our clinical team, withregular team meetings, CPD opportunities and access to further training tohelp you develop your career and skills.

Job responsibilities

JOB SUMMARY:

To deliver a high standard of patient care as an Advanced Care Practitioner - ACP, in general practice, using advanced autonomous clinical skills, and abroad and in-depth theoretical knowledge base

To manage a clinical caseload, dealing with presenting patients needs in aprimary care setting

Primary Key Responsibilities

The following are the core responsibilities of the Advanced Care Practitioner. There maybe, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

1. To manage patients presenting with a range of acute and chronic medical conditions,including undifferentiated undiagnosed conditions, providing subject matter expert advice, however the majority of your workload will be same day urgent triage.

2. To identify red flags and underlying serious pathology and take appropriate action

3. To use complex decision making to inform the diagnosis, investigation, completemanagement of episodes of care within a broad scope of practice

4. To implement and evaluate individual specialised treatment plans for chronic disease patients

5. To identify, manage and support patients at risk of developing long-term…
