Responsibilities
  • Undertake strengths-based assessments, mental capacity assessments, and reviews with individuals and their families.
  • Collaborate as part of a wider team, including occupational therapists and social workers, to develop person-centred care plans.
  • Ensure compliance with current legislation and guidance impacting adult social care.
  • Support individuals in identifying personal goals and creating strength-based plans to achieve them.
  • Develop and maintain effective relationships with service users, their families, carers, and external partners.
