Overview

As part of an exciting expansion of our Single Point of Access team, we are looking for compassionate and skilled individuals to become the first friendly voice people hear when they reach out for support. The role of Client Support & Referral Practitioner requires listening, gathering information, assessing needs, identifying risk and providing advice and guidance to help people access the right support at the right time.

Responsibilities
  • Serve as the first point of contact for children, young people, families and vulnerable adults accessing ONSIDE services.
  • Conduct triage, assessment and signposting for a wide range of ONSIDE services.
  • Gather information, assess needs, identify risk and provide advice and guidance to help people access the right support at the right time.
  • Maintain accurate records and process referrals within ONSIDE’s case management system.
  • Support parents searching for help for their child, adults seeking advocacy, families navigating challenges, and individuals looking to access neurodiversity services.
Qualifications
  • Excellent customer service skills, warm and empathetic, able to build rapport quickly while handling sensitive information with care.
  • Strong organisational skills and confidence using Microsoft Office and other ICT systems.
  • Experience supporting vulnerable children, young people, families or adults is beneficial.
  • Knowledge or strong understanding of neurodivergence and the needs of neurodivergent people is highly valued.
  • Adaptable and creative in supporting people with neurodivergence, mental ill health, disabilities or other complex needs.
