To support children and young people with disabilities and their families by assessing their needs, coordinating support and ensuring their safety, wellbeing, independence and voice are at the centre of decisions.

• Assess children and families’ needs.
• Develop and review support plans.
• Work with families and carers.
• Coordinate with health, education and social care.
• Manage complex cases.
• Make child-centred decisions.
• Promote independence and positive outcomes.
• Attend supervision and reflect on practice.
• Seek guidance when dealing with complex situations.

• Strong assessment and decision-making skills.
• Child-centred and person-centred approach.
• Good communication and relationship-building skills.

• Registered with Social Work England.
• Social Work Degree, DipSW or CQSW.
• Ability to work with children, families and partner agencies.
