Herefordshire Council is seeking a qualified social worker to support children and young people with disabilities and their families. You will assess needs, coordinate support, and ensure safety, wellbeing, independence and voice stay central to decisions.
You will work across health, education and social care, handling complex cases, developing care plans and promoting positive outcomes while engaging in regular supervision.
