NP​/PA – Family Medicine; Float Pool; Twin Ports

• Provide comprehensive care to patients of all ages, focusing on health promotion and disease prevention.
• Collaborate with physicians, advanced practice providers, and other healthcare professionals to deliver high-quality care.
• Float between multiple clinics in Northern Minnesota as needed, including Hermantown Clinic, Lakewalk Clinic, Lakeside Clinic, Proctor Clinic, and the West Duluth Clinic.
• Maintain patient-centered care and contribute to a culture of staff resilience.

Team Structure:
Work alongside a collaborative team of Physicians, Advanced Practice Providers, Pharmacists, Nurses, Clinical Assistants, and Integrated Behavioral Health professionals.

Monday – Friday, 7:00 a.m. – 5:00 p.m. (Possibility of extended hours and occasional Saturdays) FTE: 0.8

Patient Load:
Estimated 14–16 patients per day.
