Job Description:

Medical, Dental & Vision benefits paid by Employer! Up to $20,000 Sign‑On Bonus Wage Range: $45.86 – $78.02 Day Full‑time, 40hrs/week

Schedule:

Mon – Fri 8am‑4:30pm Weekend Rotation every 4 weeks Home Health & Hospice Department

Definition of Position

Assess, delegate, coordinate, and evaluate the nursing care provided. Ensure provision of quality care for selected groups of patients through utilization of the nursing process, established standards of care, policies, and procedures. Initiate the Plan of Care and necessary revisions and be responsible for the overall care of the patient. Initiate appropriate measures to prevent disease exacerbation, promote safety, make referrals to rehabilitative services, and coordinate all services provided by other disciplines/organizations.

The Home Health RN is directly responsible to the Manager of Home Health and Hospice.

Responsibilities
  • Provide essential medical care and support to patients in their home setting, helping them to recover or manage chronic conditions and related symptoms.
  • Conduct ongoing assessments of patients’ physical needs and related symptoms and develop an individualized care plan based on the patients’ needs and goals of care.
  • Maintain clear communication and collaboration with the patient, family, caregivers, and providers.
  • Communicate observations, facts, and treatment suggestions as an interdisciplinary team member, individually and through team meetings to ensure coordination of the plan of care.
  • Provide education to patients, families, and/or caregivers on disease and symptom management.
  • Maintain timely, accurate, and detailed records of patient assessments, care plans, treatment, and intervention response/outcomes.
  • Advocate and provide support for the patient.
  • Support the hospital mission, vision, values, policies, and procedures.
  • Participate in required education for DNV programs as applicable to the position (reference program education curriculum).
  • Perform other related duties as assigned.
Qualifications Education
  • Required:

    Graduate of an accredited school of professional nursing.
  • Preferred: BSN.
Licenses / Certifications / Registrations
  • Required:

    Current RN license in the State of Oregon; CPR certification; valid Oregon’s Driver’s License and proof of current auto insurance.
  • Preferred: NA.
Experience
  • Required:

    One-year continuous RN experience in an acute care setting.
  • Preferred:
    Previous Home Health experience and/or community health experience, or critical care.
Other
  • Equipment/Tools/Work Aids:
    Infusion pumps, PCA pumps, and other medical equipment, computer terminal, printer, photocopier, and patient charts.
  • Specialized Knowledge and

    Skills:

    Ability to provide care for the patient’s age‑specific, cultural, physical, and psychosocial well‑being.
Physical Requirements

The physical and sensory functions described below are essential to the successful performance of this position. The position requires sufficient auditory ability to detect and respond to various sounds, clear and effective communication, visual acuity for observing patient conditions and reading documentation, tactile perception for assessing physical characteristics, motor coordination for handling medical equipment, and the ability to lift up to 50 pounds independently.

The role involves occasional stooping, kneeling, crouching, or crawling, walking, standing, and frequently pushing, pulling, carrying, or repositioning objects or patients up to 25 pounds, and occasional lifting or transferring individuals weighing over 100 pounds. Employees may sit for long periods of time.

Working Conditions

This position is performed primarily in patients’ homes and community settings. The work environment may vary significantly from one location to another, including exposure to extreme temperatures, smoke, unsanitary conditions, structural challenges, and pets. The role involves frequent exposure to infectious diseases, hazardous materials, and medical equipment. Employees are expected to follow safety protocols and use appropriate personal protective equipment (PPE).

The environment is fast‑paced and unpredictable, requiring quick responses to changing circumstances, including high‑pressure or emergency situations. Employees may be exposed to emotionally charged situations and interactions with distressed individuals or dysfunctional family dynamics. The work may involve multiple simultaneous demands, interruptions, and varying levels of noise due to medical equipment, staff activity, or patient needs.
