Current Basic Life Support (BLS)
Experience in outpatient care, primary care, internal medicine preferred
Experience in electronic health records, clinical documentation, and outpatient procedures
Strong clinical assessment skills
Registered Nurse or Bachelors of Science in Nursing

• Provide patient-centered nursing care to support preventive health, wellness, and the management of acute and chronic conditions.
• Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.
• Perform clinical assessments, symptom triage, patient follow-up, and timely responses to patient inquiries.
• Support continuity of care through post-hospitalization, post-procedure, and other follow-up communications.
• Conduct nurse visits and assist with clinical procedures, diagnostic testing, injections, and patient education as appropriate.
• Support the use of clinical monitoring devices and technologies, as well as patient education related to their use.
• Assist with prescription refill requests, medication-related communications, and prior authorizations.
• Maintain accurate and timely clinical documentation in accordance with practice policies and regulatory requirements.
