The Health and Social Care Coordinator (HSCC) is a key member of the multidisciplinary team, responsible for supporting and coordinating weekly GP‑led HUB meetings across localities. This role is central to improving patient experience, reducing health inequalities, and supporting integrated care across health and social services.
The MDT meetings bring together professionals from various disciplines to share expertise, develop joint care plans, and ensure high‑risk and vulnerable patients receive proactive, integrated care that helps prevent crises and avoid unnecessary hospital admissions.
HSCCs act as both care coordinators and MDT facilitators, working closely with community nursing teams, GPs, and multiple KCHFT services to signpost patients and families to the most appropriate health, social, or voluntary pathways.
Main duties of the jobCore responsibilities include:
- Assessing basic equipment needs and coordinating referrals.
- Keeping the wider inter‑professional team updated to ensure timely and effective support.
- Providing trusted assessor assessments (Full training will be provided) and identifying patient needs through telephone and face‑to‑face assessments.
- Coordinating MDT hub meetings by processing referrals, gathering patient updates, signposting, taking minutes, arranging attendance of professionals, and undertaking relevant actions.
- The role requires flexibility to respond to changing priorities and autonomy in managing caseloads effectively.
