The role includes daily multidisciplinary rounds, assessments, and implementing evidence-based treatment plans for acute kidney injury, CKD exacerbations, electrolyte disorders, and hypertension.
The APP will collaborate with nephrologists, hospitalists, intensivists, nursing, and ancillary teams to ensure coordinated, patient-centered care.
