Community Link Worker

Location:

Hertford, Hertfordshire (On-site)

Salary: £26,994 per annum, pro rata

Hours:

35 per week

Contract:

Fixed term contract (until March 2027)

Additional Requirement:
Applicants must have a full licence (2 years) and access to a vehicle.

As a Community Link Worker, you'll support people facing challenges such as poor health, loneliness, housing concerns and financial pressures. You'll help them find practical solutions and connect with local services, groups and activities. You'll work closely with health, social care and voluntary sector partners to provide personalised support and prevent problems from escalating. Through home visits, community outreach and strong relationship building, you'll empower people to achieve their goals and improve their wellbeing.

What will a day in the life of a Community Link Worker involve?
  • Supporting people facing challenges with their health, wellbeing, housing, finances or social connections.
  • Helping people set goals and build the confidence to live more independently.
  • Connecting people with local groups, activities and services that meet their needs.
  • Working with health, social care, housing and voluntary sector partners to coordinate support.
  • Visiting people at home and providing practical support following a hospital stay.
  • Keeping accurate records and monitoring progress using case management systems.
To be a successful Community Link Worker, what will you need?
  • Experience supporting people in community development, health, social care, learning support or public health settings.
  • Experience working in a charity, community organisation or public sector service.
  • Strong administration skills and confidence using Microsoft Office and digital systems.
  • The ability to learn and use databases and case management systems quickly and accurately.
  • A team-focused approach and the ability to build positive working relationships.
  • An understanding of the voluntary and statutory sectors, with the ability to travel across the local area when required.
