We are seeking a dedicated Locums Internal Medicine, Family Medicine or Geriatrician Physician to provide high-quality outpatient care across multiple
Long-Term Care (LTC) and Assisted Living (ALF) facilities in Hickory, NC, Roanoke Rapids, NC and Columbia, SC areas. Thisrole focuses on delivering compassionate, patient-centered care to an aging population while supporting facility staff, patients, and families throughout the post-acute continuum.
Position Details
- Setting:
Care in multiple Long-Term Care and Assisted Living facilities - Location:
Physicians will cover multiple locations - Full and part time options available
- Schedule:
Monday-Friday, 8:00 AM - 5:00 PM - Daily Patient Volume: 18+ patient visits per day
Responsibilities
- Provide comprehensive care to residents in LTC and ALF settings.
- Manage chronic medical conditions and acute issues for geriatric and medically complex patients.
- Conduct daily patient visits, ensuring at least 18 encounters per day.
- Collaborate with nursing staff, therapists, and facility administrators to ensure coordinated care.
- Offer clinical expertise, communication, and support to patients and their families.
- Serve as an advocate and resource as patients transition through:
- Acute Inpatient Rehab
- Skilled Nursing
- Long-Term Care
- Assisted Living
- Transition back to home
- Maintain thorough documentation and follow facility protocols.
Requirements
- Board Certified or Board Eligible in either:
- Internal Medicine
- Family Medicine
- Geriatrics
- Active North Carolina or South Carolina medical license
- Experience in post-acute, geriatric, or facility-based care preferred
- Strong communication skills and ability to work well in interdisciplinary environments
- Compassionate approach to complex and aging populations
