Position: Social Worker, Acute Hospital
Schedule: Monday - Friday 8am-5pm, holiday rotation, occasional weekend work required
Shift: Full Time Days
Position Summary: Assesses and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Essential Functions- Interviews and assesses patients, families, caregivers, and legal representatives.
- Determines, prioritizes, and arranges needed internal and external services or interventions.
- Participates in case reviews to evaluate case management and progress.
- Consults with healthcare team members to promote, monitor, and evaluate compliance with the patient’s treatment plan.
- Assists with discharge planning and processes.
- Identifies appropriate resources, including transportation, housing, healthcare, and spiritual services, and provides referrals as part of the discharge plan.
- Supervises and/or trains new staff, students, and interns.
- Provides individual, family, and group therapy.
- Critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
- Ability to work in a stressful environment and take appropriate action.
- Bachelor’s degree required;
Master’s preferred. - Accepted credentials: CSW, LPCA, LCSW, RN-BSN.
- Board‑certified Social Worker or related field within 6 months of hire.
- Handle With Care certification within 30 days of hire.
