Base pay: $70,500.00/yr – $96,900.00/yr
Pay per visit/unit: $45.00 – $63.00

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management, provide necessary follow‑up as directed by the Clinical Manager.
• Confer with physician to develop the initial plan of treatment based on orders and initial assessment; provide hands‑on care, manage and evaluate the care plan, and teach the patient per physician orders under Clinical Manager supervision.
• Coordinate appropriate care with various healthcare personnel (Physical Therapists, Occupational Therapists, Home Health Aides, external providers).
• Report patient care, condition, and progress to physician and Clinical Manager continuously.
• Implement care plans in cooperation with patients and families to achieve optimal resolution of needs.
• Coordinate/oversee/supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written instructions and monitor documentation completion.
• Discharge patients after consultation with physician and Clinical Manager, preparing and completing required clinical documentation.
• Prepare medical documentation for all patients, including case conferences, medication order changes, re‑certifications, progress updates, and care plan changes.

• Diploma, Associate or Bachelor Degree in Nursing.
• Minimum of one year nursing experience preferred.
• Strong med‑surg, ICU, ER, acute experience.
• Home Health experience a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
