Position Summary

The Concerto Care Advanced Practice Provider (Nurse Practitioner and Physician Assistant) delivers in‑home clinical services to patients with complex needs, supported by a multidisciplinary care team. Our providers build meaningful relationships with patients and caregivers, champion patient‑centered strategies, and practice at the top of their license. They deliver care flexibly through a blend of in‑person visits at patients’ homes and virtual care from their own homes, with the support of nurses, community health workers, medical assistants, nurse case managers, clinical social workers, and clinical pharmacists.

Responsibilities
  • Provide compassionate, individualized, and holistic patient care, including comprehensive care, urgent care, and chronic condition management aligned with a value‑based model.
  • Collaborate with the multidisciplinary team to deliver transitions of care, social determinants of health interventions, behavioral health evaluation and management, and complex medication management as clinically appropriate.
  • Deliver clinical care both in‑person and virtually, in settings such as private homes, congregate living facilities, ambulatory clinics, and/or skilled nursing facilities.
  • Integrate clinical practices, data dashboards, and operational processes to support company performance goals.
  • Promote and participate in patient engagement and experience initiatives.
  • Maintain comprehensive medical records, including timely clinical documentation, coding, medication reconciliation, and activity logging.
  • Participate in care‑team huddles, operational huddles, organizational meetings, and clinical partnership meetings as requested.
  • Engage with internal care‑team members and external providers to support care coordination, including goals of care.
  • Adhere to medical policies, protocols, criteria, and clinical guidelines.
  • Adhere to compliance policies, procedures, and standards of conduct, including all applicable laws and regulations.
  • Serve as a growth/engagement ambassador, building relationships with patients, caregivers, facility staff, and clinical partners.
  • Participate in after‑hours on‑call schedule and other clinical care cross‑coverage as requested.
  • Perform other market‑specific duties as assigned.
Qualifications
  • Nurse Practitioner with accredited graduate education and ANCC or AANP certification; or board‑certified Physician Assistant. PMHNP or NNP certification alone does not qualify.
  • Active licensure in applicable state(s).
  • Active DEA licensure and state‑specific controlled substance registration, if applicable.
  • Advanced training in geriatrics and/or palliative medicine preferred.
  • Certification in basic life support (BLS) required.
  • Clinical experience caring for aging adults and adults with complex care needs required.
  • Experience caring for patients in the home setting preferred.
  • Experience with managed care and/or value‑based healthcare delivery preferred.
  • Average to advanced computer and software skills required.
  • Ability to travel as needed in a timely manner. Proof of liability and property damage insurance on the vehicle used is required.
  • Driver requirements: minimum 5 years’ license, no recent DUI or serious driving violations.
Vaccination Policy

Concerto Care requires all frontline workers to be fully vaccinated and to provide verification records.

Base Salary

$125,000 to $135,000 plus annual bonus. Compensation is commensurate with qualifications and may fall outside the range shown.
