- Performs accurate interqual reviews based on the physician’s order for level of care
- Notifies the PA in a timely manner if a patient does not meet admission criteria
- Enters observation hours on the Observation Log in a timely manner
- Avoidable days are documented appropriately in e/care
- Holds PCC meetings, documents team note in chart and keeps a weekly team attendance sheet
- Places a note re: the patient’s discharge plan in the multidisciplinary section at least every 3 days
- Communicates as requested to insurance companies re: patients clinical status
- Participates in the weekly Complex Case Review Meeting
- Assist SW or D/C Planner with distributing the discharge copy of the MCL, completing admission assessments and working with patients and families to plan a safe and timely discharge.
- Notify the physician to facilitate discharge when patient does not meet admission or continued stay criteria
- Associates or Bachelors degree in Nursing
- Current licensure as a Registered Nurse in the State of North Carolina
- Two years hospital based clinical experience. Experience in Case Management, Utilization Review and/or Discharge planning, knowledge of Interqual and other payor criteria preferred.
- Must have excellent communication skills, demonstrate self-direction, and have the ability to work well in a team environment.
