Licensed Placement Support Coordinator (Day shift)

Carebridge EAP is hiring for a Licensed Placement Support Coordinator (Day shift). This field-based role operates statewide in North Carolina, with travel within the assigned DSS region required and virtual work when not in the field.

Overview

We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system. The Placement Support Coordinator helps ensure children are placed in appropriate and caring foster homes and works closely with case managers, foster families, and other stakeholders to facilitate placements and provide ongoing support.

Location:

statewide in North Carolina. Residency in North Carolina is required. Travel within your assigned DSS region is required. When not in the field, you will work virtually from home. These roles are statewide and require interaction with patients, members, or providers in person four to five days per week. Travel to client sites or designated locations is required, with some office attendance for meetings or training.

Note:

Locations include travel within your assigned region; a reasonable commuting distance may be required.

What you will do (Responsibilities)
  • Perform case management telephonically and through home visits within licensure scope for programs like Foster Care.
  • Assess and identify appropriate foster home placements based on each child’s needs.
  • Develop comprehensive care plans addressing objectives and goals identified during assessments.
  • Facilitate pre-placement visits and introductions to support smooth transitions.
  • Coordinate with internal and external resources to meet integrated physical and behavioral health needs.
  • Maintain strong relationships with foster families, providing ongoing support.
  • Assist members in accessing appropriate, quality, cost-effective care and adjust plans as needed.
  • Maintain accurate records of placements and communications following organizational and legal requirements.
Qualifications
  • Active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the State of North Carolina.
  • MS/MA in social work, counseling, or a related behavioral health field, or a nursing degree.
  • Three (3) years of experience in care management, case management, or care coordination for individuals served by the child welfare system.
Preferred qualifications
  • Previous experience in foster care, social work, or related fields.
  • Strong understanding of the foster care system and child welfare policies.
  • Excellent interpersonal and communication skills and ability to work collaboratively.
  • Proficiency with Microsoft Office and experience with child welfare databases.
  • Empathy, compassion, and commitment to child welfare.
