GWM Hospital at Home Wound Care RN - Hybrid

Full‑time hybrid RN position supporting wound care and ostomy management for patients receiving Hospital at Home services, combining in‑home care with virtual assessments.

• Performs telephone triage for a diverse patient population.
• Assesses and initiates needed care, collaborating with patients and interdisciplinary teams.
• Provides comprehensive nursing assessments in an unconventional nursing environment.
• Participates in team conferences and determines patient education needs.
• Reports all patient encounters and operational concerns to leadership promptly.

• Long periods of sitting and use of a telephone headset.
• Walking, standing, lifting up to 25 lbs, reaching, stooping, bending, pushing, and pulling.
• Strong hand dexterity, sight, hearing, and concentration in a fast‑paced environment.

• Graduate from an accredited School of Nursing (BSN preferred).
• Current RN license in South Carolina and any other relevant state(s).
• BLS certification required; experience in telephone triage, pediatrics, or ED triage desirable.

Pay range: $34.90 to $52.35 per hour, based on qualifications and experience.
