Role DescriptionThe purpose of a Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health is to support our primary care providers with clinical documentation so that they can focus on providing exceptional care to our patients. Scribes assist providers throughout the patient care journey – huddling each morning to plan for the day's visits, joining them in the exam room to observe and document, and touching base after the visit to assist with next steps.
Beyond the typical Scribe role, these team members serve as Clinical Documentation Assistants (CISs), supporting accurate, specific, and timely documentation. They become experts in value‑based care, coding (ICD‑10, CPT), and care‑gap identification, receiving extensive on‑the‑job training in clinical workflows.
- Document patient encounters (~80%): observe visits, document HPI/PE/Assessment/Plan/Note, assign codes, prepare after‑visit summaries, consult with provider for accuracy.
- Clinical documentation improvement (~10%): request and review records, use population‑health tools, support daily huddles and documentation reviews, identify improvement opportunities.
- Administrative support (~10%): place orders, referrals, address tasks, support care team with documentation duties.
- Other duties as assigned.
- Medical terminology and medication knowledge (pre‑med degree or prior clinical experience) – required.
- Prior clinical experience (shadowing, volunteering) – strongly preferred.
- Prior scribe or transcription experience – preferred.
- Advanced listening and communication skills – required.
- Computer literacy and ability to learn new workflows – required.
- Fluency in Spanish, Polish, Russian or other community languages when indicated – required where indicated.
- Adapt to new workflows and learn quickly – required.
- Typing 70+ WPM – strongly preferred.
- Work in a team delivering patient care, adapt to provider style – required.
- Self‑starter within role scope.
- Excellent attendance, 40‑45 hours/week during clinic hours – required.
- Commit at least one year (2+ ideal) – required.
- Compliance with HIPAA and hospital policies – required.
- US work authorization – required.

Pay Range$17.00 – $28.46 per hour (base). Eligible for bonus, commission or short‑term incentive program.
