Position Purpose
RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an environment sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator functions as part of a team and is accountable for coordinating and delivering patient care using the Nursing Process within the organization’s standards, policies, procedures, and guidelines.
Employment TypeFull time.
Shift Schedule- Monday, Thursday, Friday: 7:30 a.m. – 4:30 p.m.
- Tuesday, Wednesday: 7:30 a.m. – 7:30 p.m.
- 1st Saturday of the Month: 7:30 a.m. – 12:30 p.m.
- Medicare annual wellness visits.
- Outreach to patients.
- Completing visits with patients.
- Escalating concerns appropriately to providers.
- Communication, education and scheduling of appointments.
- Using EPIC.
- Education:
Graduate of a school of nursing. Associate’s Degree required;
Bachelor’s preferred. - Licensure/Certification:
Registered Nurse; current, unrestricted license to practice in Ohio. - Experience:
Minimum two years in an ambulatory setting preferred; one year in case/care management preferred.
