About the RoleHome Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency and serve existing and new clients in Hillsboro and surrounding areas including Highland, Adams, Fayette, Ross, and Pike Counties. This long‑term employment opportunity can be tailored to fit your schedule: either as a second job, consistent part‑time, or full‑time with optional travel up to 1–2 hours from home.

- Hourly pay at $35/hr or salary starting at $72,800 + PTO for full‑time positions.
- Typical travel: 40‑minute radius daily (part‑time) or 60‑75 minute radius daily (full‑time); up to 1–2 hours on occasion.
- Door‑to‑door mileage reimbursement provided.
- Schedule:
Monday‑Friday, 8am‑5pm. No on‑call or overnight shifts required. - Minimum of 1 year of experience in case management; home‑health experience preferred.
- Perform client assessments and initiate appropriate actions in home‑care situations.
- Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Document, communicate, and coordinate patient status with the care team.
- Promote personal safety and maintain a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
- Manage all aspects of patient care, including SOC, recertifications, ROC, and evaluations.
- Graduate of an accredited School of Nursing.
- Minimum 1 year experience as an RN with an active Ohio license.
- CPR and First Aid certifications required.
- Experience in supervisory roles, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, confidentiality, and attendance.
- Familiarity with HIPAA regulations and medical terminology.
- Current driver’s license and reliable transportation.

- Autonomy in a client‑centered culture with quality/compliance support.
- No on‑call or overnight shifts; weekends optional.
