Travel Physical Therapist – Home Health, 13‑week travel contract with extensions available.

• Base pay: $55–$60/hour (blended rate) – weekly gross $2,200–$2,400
• Housing stipend: $850–$950/week
• Meals & Incidentals: $550–$650/week
• Overtime & Holiday rate: $30/hour
• Onboarding pay: $15/hour
• Sign‑On bonus: $500 (start within 2 weeks)
• Completion bonus: $500 (paid at end of contract)

• Provide skilled Physical Therapy services in Home Health settings.
• Perform evaluations, develop treatment plans, and document progress.
• Work independently while collaborating with interdisciplinary teams.
• Use EMR systems for accurate and timely documentation.
• Assignments may vary – flexibility required.

• Guaranteed 40 hours/week, 8‑hour shifts.
• Day, evening, or night shifts as required.
• Weekends may be required.
• Must accept assignments within a reasonable commute distance.

• Minimum 2 years of Home Health PT experience (within the last 5 years).
• Active U.S. Physical Therapist license in the state of placement.
• CPR/BLS & First Aid (AHA, hands‑on).
• Valid driver’s license & reliable personal vehicle (required).
• Strong communication, documentation, and time‑management skills.
