• Provide one‑on‑one Physical Therapy care in patients’ homes
• Perform evaluations, treatments, and progress assessments
• Create personalized care plans focused on mobility and independence
• Document visits accurately using EMR systems
• Collaborate with healthcare teams to deliver exceptional outcomes

• 5 days/week, 8‑hour shifts
• Weekdays only (optional nights/weekends with higher pay)
• Follows patient needs; flexibility required

• Weekday day shifts: $55–$60/hr
• Nights/Weekends (if applicable): $56–$61/hr
• $500 Sign‑On Bonus – start within 2 weeks
• $500 Completion Bonus – paid at end of 13‑week contract

• Active Hawaii Physical Therapist license
• 2+ years PT experience (home health strongly preferred)
• Confident working independently in community settings
• Strong documentation and communication skills
• Reliable, patient‑focused, adaptable

• AHA CPR/BLS & First Aid (hands‑on)
• TB clearance
• Immunizations/Titers
• Background check & fingerprinting
