Position SummaryResponsible for the coordination of all hospice care. Provides skilled nursing to Hospice patients utilizing nursing assessment, planning, implementation, and evaluation. Develops a plan of care for the Hospice patient and their care givers based on the nursing process and in collaboration with the physician and interdisciplinary team. Utilizes principles of palliative and Hospice philosophy to provide holistic care. Provides direct supervision of the Certified Nurse Aide.
Duties & Responsibilities- Completes the nursing assessment of the patient, including assessment of the caregiver s needs, and develops the plan of care in collaboration with the interdisciplinary team that reflects skilled nursing judgment, medical necessity, measurable patient and caregiver objectives, and reasonable goals.
- Provides nursing and case management services to patients and caregivers in compliance with federal, state, and Hospice regulations, initiating and coordinating utilization of services that best benefit the patient and the caregivers.
- Documents all nursing and case management in accordance with Hospice protocol and keeps attending physicians informed of changes in the patient s condition. Maintains documentation of time sheets, mileage, and other records as required.
- Makes regular follow-up visits, at least once per week, as deemed appropriate in the plan of care. Visits hospitalized patients daily. Responds in person, during normal working hours, to the patient s home for patient care.
- Empowers and supports caregivers and patients through education of the disease process and prescribed care regimens.
- Utilizes principles of palliative and Hospice philosophy to provide holistic care. Provides direct supervision of Certified Nursing Assistants.
- Excellent observation, critical thinking, assessment, nursing judgment, and communication skills.
- Comfortable in maintaining professional boundaries.
- Interest and desire to give hospice care with a special sensitivity to the needs of the seriously ill population.
- Sound working knowledge of hospice Conditions of Participation (COPs) and clinical standards of practice.
- Graduate of a registered nursing curriculum; BSN or higher preferred.
- Two (2) years of experience in a healthcare environment or equivalent position.
- Possession and maintenance of a valid Hawai i State nursing license.
- Valid CPR certification.
- Five days a week, primarily 8:00 a.m. to 4:30 p.m., with 1/2-hour lunch break. Duties may require after-hour work including weekends and holidays as needed.

Pay range: $95,472 - $111,914 annually (Union dues required). The actual base salary offer will depend on factors including experience, education, geography, and other relevant factors.
