We need an Occupational Therapist who can perform home, assisted living and long‑term care visits, document findings, arrange referrals, provide health education and collaborate with community resources. The role includes telehealth visits with field COTA support and travel to homes as required.

• Obtain complete health and psychosocial history and record findings systematically.
• Perform complete physical assessment.
• Arrange referrals to other health team members and community resources.
• Provide health education, counseling and community education.
• Work flexible hours and travel between offices, facilities and other locations.
• Utilize required electronic documentation.

• Successful completion of an Occupational Therapy training program and certification through the state board.
• Active license to practice in SC.
• Current CPR Certificate.
• Preferred prior experience in a healthcare setting.
• At least one year experience in the specialty field.
