Kwintes zoekt een gedreven woonbegeleider voor het team Beschermd Thuis in Gooi & Vechtstreek. Je biedt ondersteuning aan cliënten in hun thuissituatie en je werkt samen om het begeleidingsplan te realiseren.
In deze rol ben je verantwoordelijk voor dagrapportages, het onderhouden van contacten en het bieden van ongeplande zorg. Een MBO niveau 4 opleiding en relevante werkervaring zijn vereist. Het salaris ligt tussen € 2.888,- en € 3.917,- op basis van 36 uur.
