Physical Therapist – Outpatient Rehab

Role Overview: This position involves evaluating, planning, and implementing physical therapy programs to improve patient mobility, reduce pain, and prevent deformities. The therapist collaborates with physicians, develops treatment plans, and provides education to patients and families, ensuring quality care that adheres to regulatory standards.

Key Responsibilities:

• Patient Assessment: Develop and update care plans, discharge strategies, and collaborate with healthcare professionals.
• Treatment Provision: Implement and adjust therapy plans, select appropriate activities, and provide patient education and discharge instructions.
• Documentation: Record evaluations, treatment sessions, and discharge information per departmental guidelines.
• Customer Service & Professional Development: Deliver excellent service, foster teamwork, participate in ongoing training, and meet licensure and competency requirements.

Qualifications & Skills:

• Graduate from an accredited physical therapy program with a Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Hold valid licensure as a Physical Therapist and Basic Life Support certification.
• Effective communication, customer-focused approach, team orientation, and basic computer skills.
