Requirements:
Nurse Practitioner or Physician Assistant with an active North Carolina State License

Location:

• Maximum of 40 hours per week (including breaks and administrative time)
• Expected average patient load of 2 visits per hour
• Innovative, integrated use of telehealth
• Centralized care coordination, care management, and population health management
