Location: Hollywood, FL (In-Office)
Schedule: Full-Time; 9:00 am - 5:30 pm
Job SummaryWe are seeking to hire a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient s charts. You will review patient records to ensure that information is accurate, and that records and documents accurately reflect the conditions and treatment requirements of each patient.
Responsibilities- Conduct comprehensive assessments of patients medical, social, and emotional needs
- Provide case management services to ensure continuity of care and appropriate utilization of resources
- Monitor and evaluate patient progress, adjusting care plans as needed
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Overseeing patient EMR to ensure that patients receive proper care
- Complete reports to reduce patient gaps and hospitalizations.
- LPN license required
- Minimum of 2 years of experience in acute care or managed care setting preferred
- Proficient in conducting patient assessments and monitoring vital signs
- Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals
- Experience with Geriatric behavioral health preferred
- Competitive salary: $30.75 an hour- $33.00 an hour
