Coral Reef Home Health Solutions LLC is seeking compassionate, dedicated, and qualified Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing home health care team in Broward County, Florida.

Responsibilities

● Provide skilled nursing services to patients in their homes within the scope of their professional license.

● Perform patient assessments, monitor health conditions, and report significant changes to the appropriate healthcare professionals.

● Administer medications and treatments as prescribed and permitted by applicable nursing regulations.

● Provide wound care, medication management, and other skilled nursing interventions as appropriate.

● Educate patients and caregivers on disease management, medications, safety, and preventive care.

● Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated patient care.

● Develop and update nursing care plans as appropriate to the RN role; LPNs implement care under required direction and supervision.

● Complete accurate and timely clinical documentation.

● Follow agency policies and applicable state and federal regulations.

Qualifications

● Active and unrestricted Florida RN or LPN license.

● Graduation from an accredited nursing program.

● Previous home health experience preferred but not required.

● Strong clinical, communication, and organizational skills.

● Ability to work independently within the scope of licensure and applicable supervision requirements.

● Reliable transportation and willingness to travel within Broward County.

● Current CPR/BLS certification.

● Professional liability insurance and other required credentials, as applicable.
