Salary Range: $20.00-$29.00 / per hour

Resident Advocate
The Resident Advocate is responsible for fostering a climate and routines that enable residents to maximize their individuality, independence and dignity. This climate will provide residents with the highest practical level of physical, mental and psychosocial well‑being and quality of life.
Essential Duties And Responsibilities- Assists with implementing a comprehensive program which provides for counseling and other support services for residents and families in the Center
- Identifies related social needs of residents, provides appropriate services to meet the individual, as well as collective needs of residents and maintains records relating to the residents’ needs and care
- Consults with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of records
- Works directly with residents and families experiencing personal and environmental difficulties or concerns related to the resident’s physical or emotional condition
- Promotes the preservation of the resident’s physical and mental health and to prevent the occurrence or progression of personal or social problems
- Plans and implements family meetings, upon admission and thereafter, to provide a forum for ongoing discussions between resident/family and interdisciplinary team
- Acts as a liaison between residents, families, outside agencies and the Center Administrator to ensure that the resident’s rights are maintained and upheld
- Functions in a manner that adheres to all policies and procedures of the Center as a representative of the Care One Center Administration
- Documents and prepares any reports requested by Administration, also counsels and provides assistance when change in financial status occurs for residents in the Center
- Coordinates discharge planning and assists with developing an organized discharge plan for all residents
- Concerns self with the safety of all Center residents in order to minimize the potential for fire and ensures that the Center adheres to legal, safety, health, fire and sanitation codes by being familiar with his/her role in carrying out the Center’s fire, safety and disaster plans and by being familiar with current MSDS
- Puts customer service first and upholds the Care One Culture of Excellence ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights
- Bachelor’s Degree in Social Work or a human services field
- 2+ years of Social Work Experience
- Knowledge of casework processes, needs of geriatric customers, medical settings and working knowledge of funding sources such as Medicare, Medicaid and others
- Imagination, skill in working cooperatively with other professionals, staff, residents and ability to organize and carry out responsibility efficiently and effectively
- Ability to communicate effectively and appropriately both verbally and in writing
- Knowledge of governmental regulations relating to social service delivery
