Serves as a case manager for the Holmen Boys and Girls Club assisting youth who struggle with behavioral and mental health challenges. Responsibilities include working with a multi-disciplinary team to create a safe and nourishing environment where kids can grow and develop skills needed to be successful in school and in the community. The case manager will facilitate groups, develop treatment plans with identified high risk youth and work individually with some youth to enhance coping and healthy living skills.
This position will work collaboratively with the Boys and Girls Club staff and also work closely with area schools and community partners.
