Pharmacist, Per Diem

Join us as a Per Diem Pharmacist!

Hours:

Weekends required 7:00 am — 12:00 pm. As a per diem Pharmacist for Mira Vista in Holyoke, Massachusetts, you will bring your experience and knowledge where your voice matters. A Pharmacist is an integral part of our multidisciplinary treatment team. You will provide direct care and support not only to our patients but also their families, and make decisions that impact the lives of your peers, your patients, and your community.

• Ensure the delivery of appropriate medication therapy to patients in a timely and safe manner.
• Review medication orders to determine clinical appropriateness and minimize risk of adverse events.
• Contribute effectively as a member of a multidisciplinary health care team to enhance patient care.
• Supervise pharmacy technicians and pharmacy interns.
• Collaborate with all pharmacy staff members to effectively manage pharmacy inventory.
• Provide drug information to medical staff and patients upon request or when needed.
• Assist in the maintenance of relevant patient files within the Electronic Health Record.
• Comply with state and federal pharmacy regulations as well as hospital policies to ensure quality of pharmacy services.

• Graduate of an accredited college of pharmacy with a Bachelor of Science (B.S.) in Pharmacy or a Doctor of Pharmacy (Pharm.
  
  D.) degree.
• Minimum of one to three years of experience working in a hospital or long-term care pharmacy setting, strongly preferred.
• Valid Massachusetts pharmacist license.
• Experience with automated dispensing machines (e.g., Pyxis, Omnicell).
• Effective oral and written communication skills.

Compensation will be determined based on the candidate's relevant experience. $55.00 - $61.00 per hour.
