West Springfield, new 24/7, Open shifts, 7:00am - 3:00pm, 3:00pm - 11:00pm, 11pm
-7am. We are looking for a Home Care Aide to join our team! You will be directly working with a female patient, following a one-on-one care plan in clients' home, this client is getting discharged from a rehab facility next week, desperately wants to come home, she's not allowed to go home until there is 24/7 assistance in place,

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert

• Personal care Experience
• At least 2 professional work history references (not friends)
• One year prior eldercare experience
• Driver’s license, Reliable transportation, and clean motor vehicle record
• Willing to submit to criminal background checks
• Willing to accept a shift every other weekend
• Strong ability to speak, understand, read, and write in English
