Licensed Practical Nurse II – Continuous (250005Y2)
We are seeking compassionate Licensed Practical Nurses (LPNs) to provide patient‑driven care and support to our veteran population at the Holyoke Veterans Home. The role involves supervising within an assigned unit, fostering teamwork across disciplines, and maintaining compliance with nursing standards and regulations.
SchedulePart‑time (24 or 32 hours per week) shifts:
Evening (3:00 pm – 11:30 pm) and Night (11:15 pm – 7:15 am) with alternating weekend and holiday schedule.
- Provide exceptional nursing care to veteran residents by assessing health status, responding to call lights or requests for assistance, and making observations on rounds.
- Administer nursing treatments, procedures, doctor orders, and care plans; observe, identify and respond to the needs for care, including medication, equipment‑assisted care and patient/family education.
- Identify, evaluate, and assess veteran health status; confer with supervising Registered Nurses and assigned medical staff for treatment modalities.
- Recognize urgent or emergency patient‑care situations; seek assistance of the RN and/or medical staff and initiate appropriate emergency interventions as directed or needed.
- Collaborate with other resident services personnel to ensure that residents’ individual care plans are being followed.
- Establish constructive relationships with veterans and their families to promote positive relationships and learning as established by the treatment/teaching plan.
- Document incidents with or without adverse effect to the veteran and refer to RN for follow‑up and/or investigation.
- Foster the Veterans Home's mission, vision and operational policies while maintaining regulatory and department compliance with Joint Commission, and state/federal regulations.
- Maintain personal and professional growth by attending in‑services, keeping current with healthcare trends and nursing practices, and other Veterans Home committees.
- Skilled in the use of computers as needed for the completion of work assignments.
- Current, active license as a practical nurse under the Massachusetts Board of Registration in Nursing.
- CPR certification through the American Heart Association.
- Ability to follow oral and written instructions.
- Ability to deal tactfully with others.
- At least one year of experience in the care of geriatric patients in a long‑term care setting.
- Certification in dementia, hospice, or another relevant discipline.
- Ability to establish effective relationships with individuals, families, and team members.
- Demonstrated attention to detail with readiness to respond appropriately when needed.
- Capacity to maintain accurate records and exercise discretion in handling confidential information.
- Skilled in the use of a computer for completion of work assignments.
This position is an emergency personnel position. During a declared State of Emergency and in non‑emergency situations or adverse weather, staff may be required to report to work.
