- Pay or shift range: $35 USD to $42.50 USD

- Provide nursing case management services for individuals with behavioral health needs, ensuring continuity of care.
- Conduct health assessments and develop individualized care plans.
- Collaborate with interdisciplinary team members to coordinate treatment and support services.
- Monitor patient progress, adjust care plans as necessary, and ensure documentation is accurate and timely.
- Educate patients and families about treatment plans, medications, and self-care strategies.
- Facilitate referrals and linkages to community resources.
- Registered Nurse with 1+ years of experience licensed in Massachusetts (required)
- Strong assessment, documentation, and communication skills.
- Reliable transportation and willingness to travel within the service area.
- Must be at least 21

PayRange: $35 - $42.50 per hour
