Position Summary

The Care Coordinator provides participant-centered reentry, recovery support, and care coordination services through the La Nueva Vida/New Life Program, with a focus on Black and Latino men impacted by substance use and criminal legal system involvement. The Care Coordinator engages participants before and after release, connects them with treatment and community resources, and supports goals related to recovery, housing, employment, health, and successful community reintegration.

Essential Duties and Responsibilities

Participant Engagement & Care Coordination
  • Engage eligible participants before and after release through relationship-based, culturally responsive outreach.
  • Complete participant intake, assess needs, and support enrollment in reentry and recovery services.
  • Develop and regularly update individualized service plans based on participant goals, needs, and priorities.
  • Maintain consistent participant contact and provide ongoing case management and care coordination.
  • Support participants in identifying and addressing barriers to recovery, stability, and successful community reintegration.
  • Assist participants with reentry needs, including benefits activation, family reunification, community connections, and longer-term stability.
Recovery, Harm Reduction & Resource Navigation
  • Coordinate referrals and warm handoffs to substance use treatment, recovery support, harm reduction, healthcare, behavioral health, housing, employment, education, transportation, benefits, and other community services.
  • Provide overdose prevention, safer-use, and harm reduction education using nonjudgmental and participant-centered approaches.
  • Connect participants with multiple pathways of recovery and treatment based on individual needs and preferences.
  • Support workforce development, employment readiness, social skills development, and other activities that promote successful community reintegration.
  • Provide crisis support and brief interventions within the scope of the position and connect participants with appropriate services when additional intervention is needed.
Community & Program Engagement
  • Facilitate or support groups, drop-in services, communal spaces, and community-building activities as assigned.
  • Build and maintain positive relationships with community providers and resources that support participant needs.
  • Support participant engagement in recovery, employment, educational, and community-based opportunities.
  • Work collaboratively with correctional facility personnel, program staff, treatment providers, recovery organizations, and community partners.
