The Honiton Community nurisng team are seeking to recruit a Band 6 Team Lead / Specialist Nurse (Community) on a fixed term / secondment opportunity until 11/04/2027

Applicants should be a very experienced Community nurse with current clinical competencies relating to Community nursing and be able to demonstrate knowledge and understanding of current issues relating primary care, community nursing, social care and integration.

The successful candidate will oversee the designated caseloads of the team and demonstrate clinical competence developed through continual professional development, reflective practice and maintain a skills portfolio relevant to the service specification. You will undertake assessments of patients with complex health care needs and developing appropriate care plans. Monitoring and maintaining standards of care, the leadership role is to, wherever possible, maintain patients in their own preferred place of care enabling them to maximise their independence and optimise their quality of life.

You will have sound clinical leadership skills and decision-making capabilities with the ability to effectively lead and manage a team embedding safe, effective nursing care utilising care planning and risk assessing as key elements of your practice. You will be integral to the multi-disciplinary teams attending, for example:
Palliative care, Core group and Safeguarding meetings.

The successful candidate will be expected to lead in the team on EPIC documentation, planning of care, audit records using EPIC reports and communicating by electronic methods. This is a major element of documentation within this Trust. You will provide key leadership to the team in ensuring high standards, clear documentation and the use of EPIC is embedded in practice.

As part of the Specialist Nurse role you will be expected to triage effectively as part of a Triage Team using Specialist skills in initial assessment.

You will be expected to triage referrals and providing leadership and clinical expertise to a smaller group of staff working at the weekend providing essential weekend patient assessments. Linking with the Coordinating staff across Clusters to provide cross cover to ensure patient assessments are undertaken safely and effectively.

Post graduate/post registration Clinical Diploma in Advanced Clinical Practice or similar qualification or equivalent experience is essential for this post and evidence of significant experience in the community nursing setting. Evidence of a Mentorship qualification and/or evidence of knowledge and skill in the Practice Assessor role in supporting junior staff and students. You will be a Practice Assessor providing mentorship and guidance to junior staff and university nursing students undergoing placement in the team.

Working Pattern:37.5 hours per week. Monday - Friday 08:30 - 16:30
