You will round in skilled nursing and long-term care facilities alongside Ohana Psych's psychiatric APRNs, helping deliver psychiatric care to nursing home residents. This position is based in Honolulu. You prepare each facility visit, support the APRN during rounds, work directly with facility staff, and keep follow-up moving between visits. As an LPN, you provide hands-on nursing support within your licensed scope under APRN direction.
You will drive between facilities, so reliable transportation is required.
Ohana Psych and Ohana Care Clinic are clinician-led organizations serving Hawaii's behavioral health and primary care population. Ohana Psych delivers psychiatric care across the Hawaiian Islands — in outpatient clinics, nursing and long-term care facilities, and through telepsychiatry — and is expanding island by island across the state.

- Round with Ohana Psych psychiatric APRNs in skilled nursing and long-term care facilities in your assigned territory, supporting resident psychiatric visits.
- Prepare each facility visit in advance: rounding lists, census changes, new admissions and consult requests, medication changes, labs, and prior visit notes.
- Obtain and document vital signs and visit intake information; collect collateral information from facility nursing staff, social workers, and families as directed by the APRN.
- Administer and score standardized screening and rating tools (e.g., PHQ-9, GAD-7, AIMS, cognitive screens) as directed by the APRN.
- Provide practical nursing support within the LPN scope of practice under the direction of the psychiatric APRN, including medication reconciliation against the facility MAR, monitoring and reporting medication side effects, and communicating APRN orders and instructions to facility nursing staff.
- Facilitate telepsychiatry visits when the APRN is remote: set up and troubleshoot the telehealth connection, bring the resident to the visit, remain present as needed, and relay findings to facility staff.
- Serve as the day-to-day liaison between Ohana Psych and facility staff: track follow-up items, confirm orders and visit notes reach the facility chart, and coordinate scheduling of the next rounding date.
- Document accurately and timely in the Ohana Psych EHR; obtain, scan, and upload facility records needed for each visit.
- Track new consult requests from facilities and route them to the Director of Facility-Based Psychiatric Services.
- Follow facility infection-control, visitor, and safety protocols at every site, including TB clearance and any facility-specific vaccination or screening requirements.
- Maintain HIPAA-compliant handling of all patient information, including verbal, written, and electronic.
- Performs other duties as assigned consistent with the role and within scope of practice.
Graduate of an accredited practical nursing program required.
Licensure / Certification- Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Hawaii required.
- Current BLS/CPR certification required.
- Valid Hawaii driver's license, reliable personal transportation, and current auto insurance required.
- TB clearance and annual screening per facility requirements; annual HIPAA, OSHA, and infection-control training (provided by Ohana).
Minimum 1 year of LPN experience required; skilled nursing facility, long-term care, geriatric, or behavioral health experience strongly preferred.
- Comfortable, respectful communication with elderly residents, including those with dementia, psychiatric conditions, or agitation.
- Accurate vital signs, documentation, and EHR use.
- Ability to organize a multi-facility rounding day independently and keep the APRN's visit list moving.
- Professional working relationships with facility nursing staff, social workers, and administrators.
- Basic telehealth setup and troubleshooting (tablet/laptop, camera, connectivity).
