Overview
Join our team as a Case Manager for the Youth Kauhale Program, where you'll play a key role in supporting young adults experiencing homelessness on their journey to independence. You'll manage cases, coordinate with a diverse team, and ensure participants receive the services they need, including housing, medical care, and employment resources. This is a dynamic role where you'll have the opportunity to make a meaningful impact on the lives of young people while working in a collaborative and supportive environment.

Responsibilities- Conduct assessments and develop service plans for participants
- Coordinate with a diverse team to provide housing, medical care, and employment resources
- Advocate for clients in meetings and connect them to vital community resources
- Manage cases to support participants on their path to independence
- Bachelor s Degree in Social Work, Psychology, or related Human Services field
- At least one (1) year of experience with youth and youth services
- Exceptional verbal and written communication skills
- Proficiency in navigating electronic health record systems
- Consistent and timely documentation
- Access to a vehicle for transporting youth
- A valid driver s license
- Current auto insurance and safety check
