For over 45 years, we've been at the forefront of creating tailored solutions for those in crisis, offering a continuum of care that includes emergency shelters, housing programs, health services, employment assistance, and outreach.

The Veterans Services Case Manager ensures program outcomes are met and is responsible for case management duties that include intake, assessment, service plan development, short-term supportive counseling and connections to housing referrals and service coordination to meet varying needs of guests/clients served. This position serves as primary liaison and works with external service and benefit providers to ensure ongoing collaboration and consistent acquisition of community resources and support for case managed guests.

Up to $55,000 annually, actual pay commensurate with experience.

• Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
• Conduct guest (veteran and non-veteran) intakes and assessments to identify, develop and meet client outcomes; develop a transition/discharge plan for each client no longer meeting the criteria for continuing care.
• Assist guests with their service needs by maintaining a warm and welcoming attitude throughout the service episode; help clients to determine the objectives needed to reach their recovery goals and service needs which may or may not involve family and/or significant others if the guest desires.
• Provide supportive counseling, connection to provision of safe, decent and affordable housing, problem solving and skill building, service coordination for medical, mental health and substance abuse treatment needs, community service and employment assistance, and crisis support to general IHS guests including guests with veteran status residing at IHS.
• Monitor and attend essential community meetings as directed or as sponsored by external organizations serving homeless veteran and general homeless populations.

• Knowledge of community resources and veterans benefits.
• Knowledge of mental illnesses/dual diagnosis and its related issues.
• Able to work under pressure while in stressful situations and handle a high volume of cases.
• Possess personal qualities of integrity, credibility, and commitment to mission.
• Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

• High school diploma or equivalent.
• Minimum of 2 years’ experience managing small direct case management programs or support services to disenfranchised individuals or homeless services setting.
• Experience with crisis intervention and de-escalation.
• CPR and First Aid Certification (or able to complete at IHS).
• Must pass CPI (Crisis Prevention Intervention) training.

Regular full time classification. Generally Monday through Friday and some weekends. Hours and days may vary based on the needs of the consumers and organization.
