The schedule requires working three 10‑hour days and one 7.5‑hour day (37.5 hours per week). The schedule will consist of three consecutive days from 8:30 am to 7:00 pm and one day for 7.5 hours between 8:30 am‑7:00 pm. Work may include weekend days (Saturday/Sunday) and is subject to change to meet client care requirements.

Pay Range (dependent on experience): $31.54 – $38.34 per hour

The Case Manager supports the Enhanced Care Services (ECS) program within the Adult Mental Health Department. They assist clients who may be experiencing significant mental and behavioral health symptoms and may require a high level of attention and care. The Case Manager provides case management services to assigned clients, assessing eligibility, assisting clients in accessing services, and collaborating with agency professionals in case planning and service coordination.
Whateducation and experience you need
- Two (02) years of responsible experience in human services or a related field is preferred.
- Experience working with the severe and persistent mentally ill population is preferred.
- Bilingual fluency in Spanish/English is preferred.
- Must possess a valid Oregon or Washington driver’s license and be insurable for client service purposes and for travel between business offices, meetings and/or trainings.
- Must pass all criminal history check requirements as required by ORS 181.536‑181.537 and in accordance with OAR 407‑007‑0200 to 407‑007‑0370.
- Certified as a Qualified Mental Health Professional (QMHP) by MHACBO or register as QMHP‑R within 14 days of hire.
- Master’s degree in an MHACBO‑qualifying field.
- Provide case management services to all assigned clients in various community settings, considering client preference.
- Assess eligibility of potential clients, assist enrolled clients in acquiring services, and coordinate with community partners to provide a range of services and resources.
- Participate in medication appointments, medication delivery, and maintain accurate, timely documentation in client record.
- Actively participate in joint case planning and coordination with MCCFL multidisciplinary teams and agency psychiatrists.
- Meet minimum client service productivity requirements set by the agency.
- Maintain timely and coherent documentation following agency policy and support clinical software system.
- Adhere to all clinical and MCCFL program policies and procedures, and stay updated on applicable rules and OARs.
- Participate in all team and agency meetings, and attend local, regional, and/state meetings as requested.
- Perform duties independently in the community and promote a positive image of the agency.
- Maintain understanding of local behavioral health resources and MCCFL’s role in the system.
- Provide after‑hours emergency services on a 24‑hour, rotating basis if qualifications and budget allow.
- Provide administrative support such as opening/closing case files, greeting visitors, and screening telephone calls.
- Use agency vehicles for client transportation and travel between business offices.
- Maintain professional communication and confidentiality with all contacts.
- Conduct behavioral health screenings and/or mental health assessments.
- Determine and document client diagnoses.
- Assess client’s family, social, and work relationships, and provide supportive services where appropriate.
- Create and oversee an individual plan of care.
- Work three 10‑hour days and one 7.5‑hour day (37.5 hours per week) with potential weekend days; schedule subject to change.
- Provide support to populations that are elderly and/or diagnosed with Severe and Persistent Mental Illness (SPMI).
- Participate in daily psychotherapeutic groups.
