De Baron Hoofddorp zoekt een Persoonlijk begeleider (jongeren) voor 32-36 uur per week. Je werkt aan een betekenisvol leven voor cliënten met psychische kwetsbaarheid en voert individueel begeleiding uit volgens een methodisch zorgplan. Je hebt een caseload en betrekt het netwerk bij de ondersteuning.
De functie is op Hoofddorp met een jaarcontract en uitzicht op verlenging. Het salaris volgt CAO GGZ FWG
45 met max €4.273,- bruto per maand bij 36 uur.
