Zorgplein Noord zoekt een bevlogen ambulant hulpverlener voor het FACT-Jeugdteam in Hoogeveen. Je werkt samen met collega’s van diverse jeugdorganisaties in Drenthe om jongeren en hun gezin flexibel te ondersteunen.
Je levert maatwerk bij complexe vragen, werkt aan netwerken rondom de jongere en ouders/verzorgers en profileert jezelf als ambassadeur van VNN. Ervaring met verslavingsproblematiek en jongeren is vereist.
