Home Health Licensed Practical Nurse (LPN)

Make a meaningful impact each day. As a Center Well Home Health nurse, you will provide personalized, one‑on‑one care that helps patients regain independence in their homes, working closely with physicians and clinicians to develop and manage care plans that support recovery.

• Provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act, applicable licensure, healthcare standards, laws and regulations, and Center Well policies.
• Deliver skilled nursing interventions for patient illness, rehabilitation needs, and preventive care, using a holistic approach and teaching activities as outlined in the plan of care.
• Apply clinical knowledge and skills to facilitate problem resolution and individualized patient goals.
• Consult with the RN Supervisor regarding needed changes in the Plan of Treatment and accept authorized verbal orders from physicians.
• Use a systematic, goal‑driven approach to implement the nursing plan.
• Maintain timely communication with patients, physicians, clinical managers, and other team members regarding needs and status changes; participate in care coordination and discharge planning.
• Maintain professional conduct with confidential information, sharing data only when authorized and in the best interests of the patient.
• Ensure proper availability of supplies, materials, and equipment needed for safe and effective care.
• Prepare, submit, and maintain comprehensive documentation as required by the company and/or facility.

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

• Hourly: $28.00 – $40.00 per visit/unit
• Annual base pay: $44,600 – $61,400 per year

40 hours per week (full time)
