As a Mental Health Case Manager at Vail, you will:
- Provide Direct Client Support: Build meaningful, trusting relationships with individuals facing mental health challenges and offer assistance in areas like housing, financial, vocational, educational, legal, and medical needs.
- Offer Holistic Case Coordination: Ensure your clients have access to the services they need by making referrals and managing case coordination with other agencies and professionals.
- Complete Essential Documentation: You’ll complete required paperwork, including Functional Assessments, Crisis Plans, and Individual Community Support Plans (ICSPs), in compliance with Minnesota Rule 79 guidelines.
- Crisis Intervention & Support: Provide immediate support during crisis situations, helping clients navigate stressful moments and find solutions.
- Ongoing Client Interaction: Regularly meet with clients to monitor progress, make adjustments to care plans, and advocate for them in the community.
- Build Community Connections: Develop strong relationships with county and state officials, family members, employers, and other community organizations to ensure clients receive the best possible support.
You will help clients stabilize their lives, reach their goals, and achieve greater independence. Over time, you will grow personally and professionally, advancing both your skills and our organization’s mission.
What You Bring:- A Bachelor’s or Master’s degree in Human Services, Psychology, Social Work, or a related field. Social Work candidates must hold a valid and current Social Work licensure
. - At least one year of professional or internship experience in the mental health field.
- A valid Minnesota Driver’s License and access to a reliable vehicle for community-based visits (mileage reimbursement provided).
